Figgy Pudding Cup Cakes wants to create a form to sell their cup cakes online. They need a form to collect the following information:

• Customer name
• Address
• Telephone
• Email
• Pick up or delivery
• Delivery date
• Pick up date

The items they sell are:

• Red Velvet Cup Cakes
• Chocolate Sunrise Cupcakes
• Lemony Snicketts Cupcakes
• Chocolate Chip Cupcakes
• Strawberry Fields Cupcakes
• French Vanilla Cupcakes

Cupcakes must be ordered in either half dozen or dozen. The price for cupcake is $10 for a half dozen and $20 for a dozen.

They also need a confirmation page that tells customers that someone will be contacting them next day to confirm their order.

Finally they need the form by tomorrow….
